Thank you for choosing the Interventional Spine and Pain Center for your acute and chronic pain needs. You can feel confident knowing you have selected a highly qualified team of pain specialists in Manchester. New patients can learn everything they need about their first visit by reading this page. You'll find directions to our office, details about what to expect at your first visit, and our practice mission. We encourage you to print and complete the registration forms conveniently provided below in advance of your first visit to save time.

Referrals:

All new patients require a referral from their physician at the beginning of our new patient process. The referring physician will need to fax the referral form to our office along with the patient records. We will acquire all necessary medical records from other physicians. Our physicians will then review the patient information and determine whether they feel they can help the patient. Upon approval by one of our physicians, our friendly office staff will contact the patient to schedule an appointment. We do all of this as quickly as possible as we do not want patients suffering. Should immediate treatment be necessary, we will do our best to accommodate.

Upon acceptance by our physicians, New Patients, please print and fill out these forms and bring them to your first appointment. This will expedite the intake process. We look forward to serving you!

We value your time and ours and want to make your time with the physician as productive and painless as possible. For this reason we request that you please have ALL paperwork COMPLETED by the time of your appointment. You may complete it ahead of time in the comfort of your home, or arrive early enough to complete it prior to your appointment time.
